Endometriosis-Related Pain
Endometriosis is more than a reproductive health issue, it is a chronic inflammatory condition that can lead to debilitating pelvic pain and nerve sensitization. At Silver Pain Centre in Toronto, we provide personalized, OHIP-covered treatment plans that focus on managing the persistent pain associated with endometriosis, helping you regain control and improve your daily function.
What is Endometriosis-Related Pain?
Endometriosis occurs when tissue similar to the lining of the uterus grows outside the womb. This tissue responds to hormonal cycles, causing inflammation, scarring (adhesions), and significant pain that often persists even with hormonal or surgical management.
Common symptoms include:
Chronic Pelvic Pain: Persistent pain in the lower abdomen or pelvis lasting six months or longer.
Dysmenorrhea & Dyspareunia: Severe cramping during periods and pain during or after intimacy.
Nerve Sensitization: The "wind-up" of pelvic nerves that causes pain to persist even when visible lesions are removed.
Referred Pain: Pain that radiates to the lower back, hips, or legs.
Why Does Endometriosis Pain Persist?
For many patients, surgery or hormonal therapy is only part of the solution. Chronic endometriosis pain often involves complex changes in the pelvic floor and nervous system. Contributing factors include:
-
Central Sensitization: Long-term exposure to inflammatory lesions can cause the central nervous system to become hyper-responsive, meaning the body continues to “feel” pain even after the physical triggers are managed.
-
Myofascial Pelvic Floor Dysfunction: In response to chronic pain, pelvic floor muscles often enter a state of protective guarding or “hypertonicity,” leading to secondary muscle-based pain.
-
Inflammatory Adhesions: Scar tissue from previous surgeries or the condition itself can bind pelvic organs together, causing sharp or pulling pain during movement.
-
Visceral-Somatic Convergence: The nerves serving the pelvic organs and those serving the skin/muscles share pathways, leading to widespread pain that is difficult to localize.
At Silver Pain Centre, we look beyond the diagnosis to understand the specific pain generators, whether they are muscular, neurological, or inflammatory, to create a targeted intervention plan.
How We Diagnose & Treat at Silver Pain Centre
We work in conjunction with your gynecologist to address the pain symptoms that remain after standard care. Your treatment plan may include:
Trigger Point Injections: Targeting hyperactive muscles in the abdominal wall and pelvic periphery to break the cycle of muscle guarding.
Nerve Blocks (Ilioinguinal, Iliohypogastric, or Pudendal): Precise, image-guided injections to "quiet" overactive pelvic nerves.
Sacroiliac (SI) Joint Injections: Since pelvic pain often radiates to the SI joints, addressing these can significantly reduce lower back and hip discomfort.
Interdisciplinary Care: Integrating physiotherapy and chiropractic care to address postural changes and pelvic floor tension.
When to Seek Help & How We Can Support You
If your pelvic pain continues to interfere with your work, exercise, or relationships despite surgical or hormonal treatment, specialized pain management is the next step. Endometriosis-related pain is complex, and “waiting it out” often leads to further nervous system sensitization.
